Sunnyfield is looking for a Support Coordinator to join our team to focus on the Western Sydney region. In this role you will be required to support the set up and management of NDIS plans and budgets to assist participants get the best out of their plans.

You will be responsible for coordinating the right supports for participants focusing strongly on assisting them to move towards their personal goals. You will be part of a flexible and constantly changing environment, which is responsive to client needs.

Reporting to the Business Development Manager, this full time role will provide support coordination to NDIS participants.

Key responsibilities include:

  • Provide information to clients and their families on relevant disability and mainstream organisations and programs that meet their support needs
  • Qualify client eligibility to specific programs and make appropriate referrals
  • Coordinate entry, review, exit or transfer to programs and services on behalf of clients
  • Continually enhance networks with agencies and service providers to ensure clients are provided with a comprehensive range of options
  • Assist participants with plan reviews and plan budget management
  • Maintain documentation to ensure compliance with NDIS reporting requirements

To be successful in the role, you need to demonstrate the following:

  • Strong verbal and written communication skills
  • Proactive, problem-solving attitude with excellent time management abilities
  • Ability to work unsupervised within a billable hours framework
  • Relevant tertiary qualifications or equivalent experience
  • Own vehicle and current drivers licence as some travel may be required
  • Knowledge of current NDIS services and processes is desirable
